Abstract
CONTEXT:
Acute tibial shaft fractures represent one of the most severe injuries in sports. Return rates and return-to-sport times after these injuries are limited, particularly with regard to the outcomes of different treatment methods.
OBJECTIVE:
To determine the current evidence for the treatment of and return to sport after tibial shaft fractures.
CONCLUSION:
The general principles are to undertake surgical management for displaced fractures and to attempt nonsurgical management for undisplaced fractures. Primary surgical intervention of undisplaced fractures, however, may result in higher return rates and shorter return times, though this exposes the patient to the risk of surgical complications, which include surgical site infection and compartment syndrome.

Resumen
CONTEXTO:
Las fracturas agudas de la diáfisis tibial representan una de las lesiones más graves en los deportes. Las tasas de retorno y el tiempo de regreso al deporte después de estas lesiones son limitadas, particularmente con respecto a los resultados de los diferentes métodos de tratamiento.
OBJETIVO:
Determinar la evidencia actual para el tratamiento y el regreso al deporte después de fracturas de la diáfisis tibial.
CONCLUSIÓN:
Los principios generales son emprender el tratamiento quirúrgico de las fracturas desplazadas y tratar el manejo no quirúrgico para las fracturas no desplazadas. Sin embargo, la intervención quirúrgica primaria de las fracturas no desplazadas puede dar como resultado tasas de retorno más altas y tiempos de retorno más cortos, aunque esto expone al paciente al riesgo de complicaciones quirúrgicas, que incluyen infección del sitio quirúrgico y síndrome compartimental.
